Job Summary: CNY Family Care is seeking a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to support patient triage and administrative responsibilities within our dynamic practice. Following a three-month on-site training period, the role will transition to a remote work setting.
Key Responsibilities:
- Address patient inquiries and medical advice requests via the patient portal.
- Manage patient calls, providing timely triage and direct communication.
- Evaluate and respond to various patient documents, including surgical reports, imaging results, and immunization records.
- Update patient information in the Electronic Medical Record system based on document reviews.
- Communicate significant health changes to patients' Primary Care Physicians.
Qualifications:
- Current valid NYS Nurse Practitioner or Physician Assistant license.
- Minimum of two years' experience in a family practice or urgent care environment.
- Familiarity with Medent (EMR) is preferred.
- Strong knowledge of chronic disease management and critical thinking skills.
