Job Title: Licensed Practical Nurse - Long Term Care Facility
Job Summary:
MED STAFFING, INC. is seeking a skilled Licensed Practical Nurse (LPN) to join our team in a Long Term Care Facility. As a LPN, you will be responsible for delivering high-quality patient care and making clinical judgments in an effective and efficient manner.
Responsibilities:
- Utilize the nursing process to assess, plan, implement, and evaluate patient care in a Long Term Care/Assisted Living Facility.
- Perform focused interviews to identify specific patient needs.
- Assess signs and symptoms indicating physiologic and psychological changes in the patient's condition.
- Collect, analyze, and interpret data and information from healthcare members and document actual and/or potential nursing diagnoses.
- Document the patient's plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions.
- Identify and prioritize actions based on patient care requirements.
- Monitor consistency between the patient plan of care and actions of medical staff.
- Perform interventions according to identified priorities, plan of care, and facility policies and patient outcome standards.
- Provide a safe, comfortable, and therapeutic patient environment.
- Initiate and evaluate patient and/or family education.
- Revised the plan of care according to evaluation, changes in medical plan of care, and effective/ineffective nursing interventions.
- Document the patient's response to nursing and other interventions.
- Use clinical judgment in evaluation activities to meet patient care needs of an assigned floor/unit, including establishing priorities.
- Monitor safety and effectiveness of the environment and equipment.
- Participate in activities to facilitate good interpersonal communications with the chosen facility.
- Other Long Term Care (LTC) Licensed Practical Nurse (LPN) duties as assigned.
Qualifications:
- Current Licensed Practical Nurse (LPN) License for the state in which the nurse practices.
- Full command, verbal and written, of the English language.
- Current Health Certificate (if applicable)
- Current PPD or Chest X-Ray.
- One year prior Long Term Care (LTC) Licensed Practical Nurse (LPN) experience preferred.
- Current BLS card
- Successful completion of screening tests for the clinical specialty area(s) in which the Long Term Care (LTC) Licensed Practical Nurse (LPN) practices
