Key Responsibilities:
  • Develop and maintain effective relationships with patients, families, and healthcare providers to ensure high-quality patient care.
  • Process and coordinate transportation requests, ensuring timely and efficient transfers between healthcare facilities and homes.
  • Collaborate with cross-functional teams to identify opportunities for process improvements and implement changes to enhance patient care and satisfaction.
  • Monitor and analyze data to inform quality improvement initiatives and ensure compliance with regulatory requirements.
  • Provide training and support to new team members to ensure successful onboarding and integration into the team.

Essential Qualifications:
  • High School diploma or equivalent required.
  • One year of patient care experience or general customer service experience preferred.
  • Proven ability to work in a fast-paced environment with minimal supervision.
  • Demonstrated expertise in transportation coordination, logistics, and patient flow.
