**Job Title:** Primary Care Nurse

The Comprehensive Community Action Program (CCAP) is a non-profit organization dedicated to providing quality healthcare services to underserved communities. We are seeking a highly skilled Primary Care Nurse to join our team, working in a fast-paced and dynamic environment that promotes personal growth and development.
Job Responsibilities
- Provide direct patient care, including assessments, triage, and treatment planning.
- Collaborate with multidisciplinary teams to ensure seamless patient care and outcomes.
- Maintain accurate and comprehensive medical records, documenting client interactions and progress.
- Communicate effectively with patients, families, and healthcare providers, using active listening and empathy skills.
Qualifications
- State of RI RN license
- Ambulatory healthcare experience
- HIV certification desirable
- Effective communication and collaboration skills
- Ability to work independently and as part of a team
