Job Title: Field Registered Nurse Assessor

Maximus is seeking a highly skilled and experienced Field Registered Nurse Assessor to join our team. As a Field Registered Nurse Assessor, you will be responsible for conducting home assessments in the Bronx, New York, to determine eligibility for community-based long-term services.

About the Role:

This is a full-time position that requires attending and conducting assessments in consumer homes, facilities, or through a telehealth modality, depending on operational need. You will work closely with the clinical leadership team to conduct home assessments and provide outstanding care to vulnerable populations.

Key Responsibilities:
  • Conduct in-home, in-facility, or telehealth assessments to evaluate individual's health status, strengths, care needs, and preferences using the UAS-NY assessment tool.
  • Establish and maintain positive relationships with consumers, caregivers, and facility staff.
  • Regularly review medical documentation, reporting any concerns, conflicting information, or issues that may arise during the evaluation process.
  • Enter assessment data into the UAS-NY online application and transmit, as required.
  • Code the UAS assessment correctly based on training and guidelines in the UAS-NY Community Health Assessment Reference Manual.
Requirements:
  • Current Registered Nurse (RN) license valid in the state of practice is required.
  • Bachelor's degree and 5 years of experience or equivalent combination of education and experience required.
  • Minimum of 5 years of clinical experience.
  • Minimum 1 year supervisory experience preferred.
