Job Title: Pharmacist

We are seeking a highly skilled and experienced Pharmacist to join our team at Bon Secours Community Hospital. As a Pharmacist, you will play a critical role in ensuring the safe and effective use of medications in our hospital.

Responsibilities:
  • Review medication orders for appropriateness and accuracy, including contraindications, allergies, dose, route, frequency, and duplicate therapy.
  • Interpret, clarify, and rectify physician medication and IV orders, including the medication reconciliation process for inpatients and outpatients.
  • Contact physicians to verify patient medication orders for dose, route, name, and frequency of administration.
  • Prepare IV admixtures, including chemotherapy, in accordance with established aseptic methods.
  • Compound sterile and non-sterile preparations in accordance with compounding regulations.
  • Monitor patients on specified target drugs and initiate pharmacokinetic dosing protocols.
  • Evaluate drug therapy for appropriateness and make drug therapy recommendations to clinicians.
  • Prescribe drug therapy per collaborative drug therapy management agreements and Medical Executive Committee approved protocols.
  • Dispense the final product for administration, instruct patients on the proper use of medications, and perform patient counseling.
  • Identify, monitor, and report adverse drug events and medication errors in accordance with established procedures and regulatory requirements.
  • Supervise and train Pharmacy Technicians to ensure accurate medication inventory, preparation, and distribution.
  • Participate in the development of drug protocols and clinical pathways.
  • Perform drug usage evaluations on selected drugs.
  • Complete quality assurance auditing as required for prescriptive authority.
  • Utilize automated medication dispensing machines.
  • Dispense controlled substances and maintain proper distribution and inventory records in accordance with established procedures and regulatory requirements.
  • Purchase medications in accordance with contractual agreements.
Qualifications:
  • Hospital experience or completion of an approved Pharmacy internship in a hospital or Master's Degree in Pharmacy Internship in Hospital Pharmacy required.
  • Prior experience with CPOE system preferred.
  • Bachelor's or higher degree from an accredited Pharmacy Program.
  • Possession of a valid license and current registration as a Pharmacist issued by the State of New York.
