Job Summary

We are seeking a highly skilled and experienced Lead Pharmacist to join our team at Bon Secours Community Hospital. As a key member of our pharmacy department, you will be responsible for ensuring the optimal use of medications to achieve specific outcomes that improve patient quality of life.

Responsibilities
  • Review medication orders for appropriateness and accuracy, including contraindications, allergies, dose, route, frequency, duplicate therapy, and formulary status.
  • Interpret, clarify, and rectify physician medication and IV orders, including the medication reconciliation process.
  • Prepare IV admixtures, including chemotherapy, and monitor patients on specified target drugs.
  • Initiate pharmacokinetic dosing protocols, evaluate drug therapy for appropriateness, and make drug therapy recommendations to clinicians.
  • Prescribe drug therapy per collaborative drug therapy management agreements and Medical Executive Committee-approved protocols.
  • Supervise and train pharmacy technicians to ensure accurate medication supply and supervise and train pharmacy interns and externs.
  • Participate in the development of drug protocols and clinical pathways, perform drug usage evaluations, and complete quality assurance auditing as required for prescriptive authority.
Requirements
  • Hospital experience or internship in hospital pharmacy, with prior experience with EPIC CPOE System preferred.
  • Possession of a valid license and current registration as a Pharmacist issued by New York State.
  • Bachelor of Science required, Doctorate in Pharmacy or Master's preferred.
