We are seeking a skilled and compassionate Patient Care Manager to join our team at SouthernCare, part of the Gentiva family of hospice, home health, palliative, and community care providers.

About the Role
The Patient Care Manager will be responsible for the day-to-day operations of an assigned hospice site, ensuring the provision of quality care to patients. This includes administering the clinical aspects of the hospice program, managing site operations in the absence of senior staff, and overseeing clinical and administrative functions.
Responsibilities
- Manage and oversee clinical and administrative functions
- Respond to phone referrals and inquiries in a timely and professional manner
- Participate in on-call nursing schedule and provide direct skilled bedside nursing care in patients' homes
- Collaborate with multidisciplinary teams to deliver patient-centered care
- Ensure compliance with regulatory guidelines and managed care principles
Requirements
To be successful in this role, you will need:
- A minimum of three years' experience in direct patient care in a clinical setting
- A minimum of one year of management or supervisory experience
- Two or more years' experience in hospice or home care
- A current license to practice nursing in the state of employment
- CPR certification and valid driver's license

Estimated Salary Range
$85,000 - $110,000 per year, depending on location and experience.
