Job SummaryWe are seeking a highly skilled Patient Care Manager to join our team. As a key member of our leadership team, you will be responsible for overseeing the day-to-day operations of your assigned hospice site, ensuring the provision of quality care to patients, and promoting a positive work environment.
Main Responsibilities- Manage the site in the absence of the Executive Director or Senior Patient Care Manager, demonstrating strong leadership and decision-making skills.
- Oversee clinical and administrative functions, ensuring compliance with regulatory guidelines and managing records effectively.
- Respond promptly and professionally to phone referrals and inquiries, providing compassionate support to patients and their families.
- Participate in the on-call nursing schedule and provide direct skilled bedside nursing care in patients' homes, showcasing expertise and dedication to patient care.
To succeed in this role, you must possess:
- A minimum of an Associate's Degree in Nursing from an accredited school of nursing, with a Bachelor's Degree in Nursing preferred.
- Three years of experience in direct patient care in a clinical setting, with a minimum of one year of management/supervisory experience.
- Two or more years of experience in hospice or home care, with a current license to practice nursing in the state of employment.
- CPR Certification and valid automobile insurance.
You should demonstrate:
- Sensitivity to the feelings of terminally ill patients and their families, with the ability to communicate effectively across socioeconomic backgrounds.
- Flexibility in performing varied tasks, adapting to changing situations without compromising efficiency or composure.
- Tactful interaction with colleagues and members of the hospice team, displaying maturity, cooperation, and flexibility.

Based on industry standards and the level of responsibility associated with this position, we estimate the salary range to be between $65,000 and $85,000 per annum, depending on qualifications and experience.
