Overview:

The Community Council of Nashua, Inc. is seeking a dedicated full-time Clinician to be part of our Emergency Services Division. This role involves collaborating with a multidisciplinary team, including first responders and healthcare professionals, to manage psychiatric emergencies, prevent hospitalization or incarceration, and facilitate immediate community-based care. The Clinician will conduct assessments in various environments, including homes and healthcare facilities, and provide therapeutic interventions to individuals and families in crisis, aiming to mitigate risks such as suicide and violence while reducing the necessity for hospitalization.

Key Responsibilities:

  • Conducts emergency crisis evaluations and fulfills all service delivery requirements for diverse populations, including youth, adults, and seniors across multiple environments.
  • Employs a range of Evidence-Based Practices (EBPs) and assessment tools for effective risk evaluation and crisis management.
  • Independently formulates and initiates action plans in collaboration with clients, psychiatrists, case managers, and other relevant personnel.
  • Provides crisis consultation and intervention support to internal programs, enhancing treatment planning and support.
  • Delivers assessment, therapeutic interventions, and short-term crisis counseling, alongside case management services.
  • Evaluates clients' crisis conditions, identifies immediate needs, and determines the appropriate intervention strategies based on risk factors.
  • Manages acute behavioral health crises, performing assessments, triage, and referrals to suitable services.
  • Establishes and maintains collaborative relationships with hospital staff and other service providers to ensure comprehensive care for clients in crisis.
  • Stays informed about internal and external resources, policies, and compliance requirements to deliver high-quality care in dynamic settings.
  • Responds to phone inquiries, providing timely assessments and determining subsequent steps for clients with behavioral health concerns.
  • Demonstrates willingness to transport clients when necessary.

Qualifications:

  • Master's Degree in Clinical Mental Health Counseling, Social Work, or a related discipline.
  • 1-2 years of relevant experience in the field.
  • Ability to quickly establish rapport and engage with a diverse range of clients, including those with severe and chronic conditions.
  • Strong collaboration skills, with the ability to work effectively both independently and as part of a team.
