Job Summary

We are seeking a highly skilled Behavioral Health Clinician to join our team at Dartmouth Health. As a key member of our primary care team, you will provide high-quality behavioral healthcare services to patients in need.

Responsibilities
  • Collaborate with primary care teams to address behavioral health needs of patients using the Collaborative Care model.
  • Conduct triage visits and initial evaluations to clarify behavioral health concerns and develop individualized care plans.
  • Enroll patients in the collaborative care program, including in-person and telephone outreach visits.
  • Utilize evidence-based clinical procedures to deliver care and measure patient symptoms and response to treatment.
  • Effectively maintain and manage patient registries to track progress and report program metrics.
  • Provide limited face-to-face, short-term counseling, including problem-solving therapy, brief CBT, and motivational interviewing.
  • Communicate with patients, primary care team members, and consulting psychiatrists to ensure effective, patient-centered care.
  • Assist patients in need of higher levels of care or specialty services to connect with appropriate treatment resources.
Qualifications
  • Master's degree in Social Work, Mental Health Counseling, Marriage & Family Counseling/Therapy, or Addiction Counseling from an accredited institution.
  • Current licensure as a Licensed Independent Clinical Social Worker (LICSW) or Licensed Clinical Mental Health Counselor (LCMHC).
