Position Overview:

The primary objective of this role is to assess, develop, and coordinate care management strategies tailored to the individual needs of members, ensuring optimal healthcare outcomes that are both effective and economical. This includes creating or contributing to personalized care plans and educating members and their families on available services and benefits to enhance healthcare access and quality through advocacy and coordinated care.

Key Responsibilities:

  • Support Services: Provide psychosocial and resource assistance to members and caregivers, facilitating access to local services such as employment, education, housing, and healthcare based on comprehensive service assessments.
  • Needs Evaluation: Assess member needs, identify barriers to care, and recommend actionable plans to achieve the best possible outcomes.
  • Care Coordination: Act as a liaison between members, families, and healthcare teams to ensure timely access to necessary services.
  • Monitoring Progress: Track advancements towards care plan goals and collaborate with healthcare providers to adjust plans as needed based on member status or changes in condition.
  • Documentation: Collect and maintain accurate member information and care management activities to comply with all relevant regulations.
  • On-Site Assessments: Conduct visits to evaluate member needs and collaborate with providers or resources as necessary.
  • Education: Provide training to care managers and members on healthcare procedures, provider instructions, and available care options.
  • Additional Duties: Perform other responsibilities as assigned to meet the needs of members and the organization.
  • Policy Compliance: Adhere to all organizational policies and standards.
Qualifications:

A Bachelor's degree along with 2 to 4 years of relevant experience is required. Candidates with a clinical nursing license from an accredited institution are preferred.

Licensure:

Current clinical licensure in the respective state is preferred.
