The Department of Veteran Affairs is seeking a skilled Physician to join our team in a Geriatrics Consultation and Geri PACT Clinic in Martinez, CA. As a Geriatrician, you will play a crucial role in providing patient care management in an interdisciplinary Geriatrics Clinics.

Key Responsibilities:

  • Lead the management of a proportional panel of geriatric primary care patients with complex medical conditions and dementia.
  • Collaborate with an interdisciplinary team consisting of a Geriatrician, Nurse Practitioner, Registered Nurse, Dietician, Pharmacist, and Social Worker to evaluate and manage patients.
  • Develop and implement treatment plans to improve patient outcomes and reduce hospital re-admissions and Emergency Department visits.
  • Participate in the development and implementation of the Geriatrics Section's mission and goals.

Work Environment:

The Geriatrics Section is organizationally aligned under the Geriatrics and Extended Care (GEC) Service. The successful candidate will work in a dynamic and supportive environment, with opportunities for professional growth and development.

Requirements:

  • Board certification in Geriatrics or a related field.
  • Experience in patient care management in an interdisciplinary setting.
  • Strong communication and collaboration skills.
  • Ability to work effectively in a team environment.
