We are seeking a dedicated and compassionate Geriatrics physician to join our team at the VA Northern California Health Care System in Martinez, CA.
Job Summary: The successful candidate will provide patient care management in an interdisciplinary Geriatrics Clinics, serving elderly Veterans with complex geriatric medical conditions and dementia.
Responsibilities:
- Lead the Interdisciplinary Geriatrics team in the East Bay to achieve the Service's mission and goals.
- Manage a panel of geriatric primary care patients with complex needs.
- Collaborate with a core interdisciplinary team consisting of a Geriatrician, Nurse Practitioner, Registered Nurse, Dietician, Pharmacist, and Social Worker.
Work Environment: The Geriatrics Section is organizationally aligned under the Geriatrics and Extended Care (GEC) Service. The work schedule is Monday - Friday, 8:00am - 4:30pm, with possible one day telework granted depending on provider availability.
