Dedicated Home Health Nurse

We are currently seeking a dedicated and compassionate Licensed Practical Nurse (LPN/LVN) to join our team as a home health nurse. This is an exciting opportunity for individuals who want to make a meaningful difference in people's lives while enjoying a better work-life balance.
About the Role:
This position offers the chance to provide skilled nursing care to patients of all ages, with a focus on pediatrics. As a home health nurse, you will be responsible for delivering quality patient care, under the supervision of a Registered Nurse, in accordance with the developed care plan and physician orders.
Key Responsibilities:
- Deliver and coordinate quality patient care in compliance with physician orders
- Continuously observe and assess patient condition and care needs, reporting changes to the supervisor and/or physician as necessary
- Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner whenever care is delivered
- Participate in and update the nursing care plan
- Take appropriate nursing action based on assessment and achieve expected outcomes
- Recognize changes in patient needs and responses requiring intervention and implement care to prevent risk or reduce risk
- Accept responsibility for personal and professional accountability by complying with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act
- Provide care utilizing infection control measures that protect both staff and patient according to OSHA standards
- Educate patients and families regarding disease processes, self-care techniques, and prevention strategies, and in meeting patient nursing needs
- Maintain knowledge of competencies related to the nursing profession by participating in educational programs, continued education units, internal learning management skills, and skill evaluations
Requirements:
- Graduate of an accredited school of nursing
- Current, unrestricted state license as a Licensed Nurse in the state of practice
- Current CPR certification
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures
Preferred Qualifications:
- Six (6) months of recent experience as a Licensed Nurse in a clinical care setting
- Home health experience

Physical Requirements:

- Must have visual and hearing acuity
