LPN - Clinic II (Meds) - Healthcare Professional

As a Licensed Practical Nurse (LPN) in our Clinic II (Meds) team, you will provide direct and indirect nursing care to patients in an outpatient setting. Under the direction of a Registered Nurse and medical orders from a Practitioner, you will be responsible for reviewing and preparing medical records, initiating follow-up for medical records, and identifying and preparing orders for patient needs.
Key Responsibilities:- Medical Record Management: Review and prepare medical records for assigned patients, initiate follow-up for medical records as requested, and review patient needs to identify and prepare orders for patient care.
- Basic Nursing Procedures: Implement basic nursing procedures per plan of care, administer treatments, immunizations, and medications as prescribed by a Practitioner, and prepare medications for Practitioners to use.
- Documentation and Communication: Document implementation of orders/interventions, record findings, instructions, and patient responses on appropriate forms or online records using department and/or JCAHO standards, and observe patients/family responses to plan of care.
- Collaboration and Teamwork: Collaborate with RN/Practitioner to revise care plan when needed, and participate in staff meetings as needed.
- Emergency Response: Perform efficiently in emergency patient care/clinic situation, follow established procedures, and inform appropriate Practitioner/RN.
- Immunization Management: Manage all patient vaccine data, including updating Hawaii Immunization Registry System (HIR), Electronic Health Records (EHR), and all patient and/or external records at each patient visit.
- Inventory Control: Facilitate vaccine Health Plan and VFC inventory control at assigned clinic, ordering vaccine and maintaining appropriate vaccine inventories.
- Training and Development: Train new and existing LPNs on emergency vaccine storage procedures, and participate in orientation of new personnel.
Requirements:
- Education: Current LPN licensure in the state of practice.
- Experience: N/A
- Certifications: Certification in Spirometry, Hearing Conservation, & Substance Abuse Specimen Collection or acquire certification within one year of hire.

- Work Environment: Clinic II (Meds) team in an outpatient setting.
- Physical Demands: Ability to lift, carry, push, and pull equipment and supplies, and ability to stand for long periods of time.
