Job Summary:

As a Licensed Practical Nurse (LPN) at Kaiser Permanente, you will play a vital role in providing high-quality patient care in our outpatient setting. Under the direction of a Registered Nurse and medical orders from a Practitioner, you will be responsible for delivering direct and indirect nursing care to patients. Your expertise will be essential in ensuring the well-being and safety of our patients.

Key Responsibilities:
  • Review and prepare medical records for assigned patients, ensuring accuracy and completeness. Initiate follow-up for medical records as requested and review patient needs to identify areas for improvement.
  • Implement basic nursing procedures per plan of care, administering treatments, immunizations, and medications as prescribed by the Practitioner. Prepare medications for Practitioners to use and perform point-of-care testing as authorized.
  • Document implementation of orders/interventions, recording findings, instructions, and patient responses on appropriate forms or online records. Observe patients/family responses to the plan of care and report unexpected responses to the RN or Practitioner.
  • Collaborate with RN/Practitioner to revise care plans as needed and demonstrate competency in basic nursing skills and equipment usage. Maintain current knowledge and skills necessary to perform the position.
  • Perform efficiently in emergency patient care/clinic situations, following established procedures and informing appropriate Practitioner/RN. Assign basic nursing tasks to unlicensed personnel as appropriate and according to department/unit standards.
  • Follow discharge plans developed by Practitioner or RN, process and/or follow-up with referrals as directed by Practitioners. Provide instruction and pre-printed information to patients as directed by RN and/or Practitioners.
  • Manage all patient vaccine data, including updating the Hawaii Immunization Registry System (HIR), Electronic Health Records (EHR), and all patient and/or external records at each patient visit.
  • Identify patient immunization requirements in accordance with age-specific guidelines per CDC. Fill out appropriate immunization paperwork for recording purposes and facilitate vaccine Health Plan and VFC inventory control at assigned clinics.
  • Utilize file maintenance to ensure newly arrived vaccines are entered in the I lot manager system as needed. Ensure immunization clinics comply with health plan, federal, and state guidelines.
  • Train new and existing LPNs on emergency vaccine storage procedures and serve as a liaison with the state to coordinate VFC (vaccines for children) inventory.
  • Assist in ensuring compliance with infection control policies and protocols and follow medication administration, storage procedures, Health Plan CDC, State, and federal VFC program guidelines.
  • Participate in orientation of new personnel, demonstrating sufficient knowledge, skills, and abilities in the area of specialization. Perform clean/sterile technique, set up for procedures, assist with clinical procedures within department scope, and follow the recommended process for handling point-of-use instruments as outlined by the High-Level Disinfection Committee.
  • Maintain, order, organize, rotate, and stock supplies. Schedule patients for routine care, acute care, and addressing care gaps. Prepare and process patient forms.
Requirements:
  • Hawaii State Board of Nursing-approved curriculum for LPN licensure.
  • Licensed Practical Nurse (Hawaii) required at hire.
  • Basic Life Support from American Heart Association.
  • Demonstrated knowledge of and skill in conflict resolution, customer service, interpersonal relations, oral communication, problem-solving, teamwork, and written communication.
Preferred Qualifications:
  • Previous nursing experience.
  • Previous clinic experience.
  • Demonstrated knowledge of and skill in word processing and spreadsheet PC applications.
