As a RN Clinical Liaison, you will play a critical role in ensuring the intake and referral processes are performed effectively and timely to meet the needs of patients, families, and referral sources.
Key Responsibilities:- Ensure accurate and timely information regarding hospice services is provided to anyone inquiring
- Receive calls from referral sources, respond timely, and initiate paperwork to begin Intake/referral process
- Serve as patient advocate in evaluating/developing plan of care and obtaining informed Consent for hospice
- Calling on multiple account types – SNF's, ALF's, Hospitals and Physicians
- Offering in-services and education on the benefits of hospice
- Hospice heart: compassionate, kind, and empathetic to your clients, their patients, and their families.
- RN license in the state(s) you will be employed
- Minimum of 2 years of healthcare experience preferred in one of the following settings: hospice, home health, clinical liaison, RN liaison, clinical coordination, discharge planner, hospice case manager, or RN educator
- Known and respected within your community
