Job Overview

Schedule: 40 hours per week; Tuesday-Saturday, 8:00am-4:30pm.

Pine Street Inn's Permanent Supportive Housing Department offers comprehensive and intensive case management services to individuals experiencing homelessness and chronic homelessness, utilizing the Housing First approach to foster long-term housing stability and combat homelessness in the community.

The Case Manager is an integral member of a dedicated case management team, assisting tenants who may face untreated mental health issues, substance use disorders, and other significant health challenges. Employing a Harm Reduction and Trauma-Informed framework, the Case Manager engages with participants through established methodologies such as Stages of Change Theory and Motivational Interviewing techniques.

Key responsibilities include managing a designated caseload of tenants and collaborating with fellow housing staff to enhance program-wide initiatives aimed at stabilizing and promoting the personal development of all residents supported by Pine Street Inn. Additional case management duties encompass:

  • Facilitating tenants in completing necessary program documentation.
  • Assisting tenants in accessing benefits, healthcare, educational resources, employment opportunities, and legal assistance.
  • Supporting tenants in developing daily living skills and crafting individualized service plans while maintaining all required documentation.

Qualifications:

EDUCATION/TRAINING:

  • High School Diploma or equivalent.
  • Proficiency in computer applications, particularly Microsoft Word, Outlook, and Excel.

PREFERRED:

  • College degree.

KNOWLEDGE/EXPERIENCE:

  • Demonstrated commitment to applying Harm Reduction and Motivational Interviewing techniques to enhance tenants' behavioral health.
  • Familiarity with effective Crisis Intervention strategies and the ability to respond effectively in crisis situations.
  • Experience working collaboratively within a team environment.

PREFERRED:

  • Experience supporting individuals facing substance use issues, mental health challenges, trauma, homelessness, or other complex conditions.
  • Experience in Housing First programs, particularly with chronically homeless individuals.
  • Knowledge of local resources for caregiving and advocacy for individuals with substance use disorders, mental health issues, trauma histories, and other related challenges.
  • Understanding of government benefits systems and local service providers.
  • Familiarity with Homeless Management Information Systems (HMIS).
