Job SummaryWe are seeking a highly skilled and compassionate Nurse Practitioner/Physician Assistant to join our team. As a key member of our patient care team, you will be responsible for providing primary and specialty care to patients of all ages, from pediatrics to geriatrics. You will work collaboratively with our multidisciplinary team to develop and implement individualized care plans that address the unique needs of each patient.
Responsibilities- Provide comprehensive patient assessments, diagnoses, and treatment plans
- Order and interpret diagnostic tests, including lab work and imaging studies
- Prescribe medications and therapies as needed
- Develop and implement individualized care plans that address the physical, emotional, and social needs of patients
- Collaborate with other healthcare professionals to ensure seamless care coordination
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
- Graduate of an accredited Nurse Practitioner or Physician Assistant program
- Current licensure and certification in the state of Iowa
- Experience working in a primary care setting, preferably in a community health center
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced, dynamic environment
