Job Summary

This position is for a case manager with the Access Transformative Outreach Program (ATOP) at Denver Health Outpatient Behavioral Health Services (BHS). ATOP supports individuals with a primary substance use disorder diagnosis and provides assertive multidisciplinary care that is community-based.

Key Responsibilities
  • Provides individual, family, and group psychotherapy to address identified mental health and/or substance disorders under the supervision of a licensed therapist.
  • Documents therapy treatment plans, interventions, and client response to treatment according to State guidelines.
  • Identifies patients' conditions, problems, and determines diagnosis.
  • Evaluates emotional components, impacts of health issues, and assists clients to identify strengths, dysfunctional factors, and establish detailed and coordinated treatment plans.
Requirements
  • Masters degree in counseling or a related field.
  • 1-3 years of experience working in mental health and/or substance use disorders.
  • Experience with client assessment and providing individual/family psychotherapy, group therapy, and documenting clinical progress.
