Job SummaryWe are seeking a highly skilled Speech Language Pathologist to join our interdisciplinary team. As a Speech Language Pathologist at Morris View Healthcare Center, you will have the opportunity to work with a diverse range of patients and contribute to their rehabilitation and recovery.
Key Responsibilities- Assessment and Evaluation: Conduct thorough assessments of residents' speech, language, cognitive, and swallowing abilities to identify areas of strength and weakness.
- Individualized Care Plans: Develop customized intervention plans to address communication and swallowing difficulties, taking into account each resident's unique needs and goals.
- Section GG Data Collection: Ensure accurate and consistent collection of Section GG data for Annual, Quarterly, Falls, and Significant Change assessments, Section K, BIMS, PhQ2-9, and other clinical data as applicable.
- Treatment: Administer speech therapy interventions, including exercises, communication strategies, and swallowing therapy, to help residents achieve their maximum potential.
- Documentation: Maintain accurate and timely documentation of resident progress and any interventions provided, using Point of Service (PoS) care delivery and documentation systems.
- Education: Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress, and conduct remote check-ins with discharged patients to ensure a smooth transition to home or next level of care.
- Interdisciplinary Collaboration: Collaborate with other rehab staff, such as Physical Therapists and Occupational Therapists, to provide holistic care for residents and promote a culture of teamwork and communication.
