Key Responsibilities
The primary role of the After-Hours Registered Nurse is to ensure the comprehensive and timely provision of hospice services for patients and their families outside of regular business hours, including evenings and weekends.
- Assess patient needs via phone triage until an in-person visit can be arranged.
- Respond promptly and effectively to urgent calls from patients and families after hours.
- Conduct initial nursing evaluations to determine eligibility for hospice services during patient visits, within a specified timeframe of referral if assigned.
- Capable of performing after-hours visits, including admissions, death visits, recertifications, and routine assessments.
- Identify and reassess the physical, psychosocial, emotional, and environmental needs of patients and families at least every fifteen days.
- Document issues, set appropriate goals, implement interventions, and record patient/family responses to hospice care.
Qualifications
Experience Requirements
- Minimum of one year of clinical experience.
License Requirements
- Valid RN licensure in the state of practice and one year of clinical experience.
- Current CPR Certification.
- A valid driver's license, vehicle insurance, and access to reliable transportation or public transit.
