Key Responsibilities- Perform comprehensive nursing assessments on patients within 8 hours of admission, identifying teaching needs and goals related to their diagnosis.
- Develop and implement individualized plans of care, collaborating with patients, families, and the interdisciplinary team.
- Provide timely and accurate documentation of patient assessments, interventions, and responses in the medical record.
- Participate in patient rehabilitation programs, utilizing the nursing process to assess, plan, implement, and evaluate patient care.
- Initiate discharge planning and patient teaching, ensuring a smooth transition to post-acute care.
- Communicate effectively with healthcare providers, patients, and families, promoting continuity of care and patient safety.
- Supervise and train ancillary nursing personnel, delegating tasks and responsibilities as needed.
- Current licensure as a Registered Nurse in the state of practice.
- Experience in patient care, preferably in a hospital or healthcare setting.
- Strong communication and interpersonal skills, with the ability to work effectively in a team environment.
- Ability to prioritize tasks, manage time, and maintain a safe and organized work environment.
