POSITION SUMMARY:

The Integrated Care Management Nurse is a vital member of the One Health team, bringing specialized expertise to enhance the organization's mission. Under the guidance of One Health leadership, this role requires a proactive approach to provide specialized nursing services aimed at improving patient health outcomes.

Coordinator Responsibilities:

  • Utilizes advanced knowledge to oversee organizational projects and programs.
  • Operates independently while collaborating with both internal and external stakeholders.
  • Facilitates the development and maintenance of initiatives within service lines.
  • Engages with teams across the organization to ensure effective communication and collaboration.

Position Specific Duties:

  • Provides comprehensive care management services in partnership with provider teams and other healthcare professionals.
  • Develops personalized care plans for patients and ensures they are well-informed about their health management.
  • Identifies patients needing care management services and ensures compliance with documentation standards.
  • Educates patients using clear, accessible language to promote understanding.
  • Refers patients to community resources and advocates for their needs while maintaining professional relationships.
  • Conducts laboratory tests and provides counseling before and after examinations.
  • Identifies patient needs and coordinates appropriate referrals.
  • Maintains accurate and organized patient records in accordance with clinic policies.
  • Collaborates with clinical teams to enhance care delivery and expand management services.
  • Participates in value-based care initiatives related to care management.
  • Trains clinic staff in care management practices and supports team management.
  • Demonstrates a patient-centered approach throughout the patient journey.
  • Adheres to all organizational policies and participates in risk management activities.
  • Provides exceptional customer service, embodying the mission of One Health.
  • Performs additional duties as assigned.

MINIMUM QUALIFICATIONS:

Education/Certificates/Licenses:

  • Nursing degree from an accredited institution.
  • Current nursing license in the state of practice.
  • Valid driver's license.
  • BLS certification upon hire.

Experience:

  • Minimum of one year in community health or related fields.
  • Preferred: Two years in community health or physician's office settings.
  • Preferred: Two years of experience in care management.
