Job Summary:

Capstone at Station Camp is seeking a skilled LPN to provide high-quality nursing care to our residents. As a Nursing Care Specialist, you will be responsible for delivering comprehensive health and psychosocial evaluations, monitoring resident conditions, and coordinating services to ensure the best possible outcomes.

Responsibilities:

• Provide nursing care in accordance with resident plans of care
• Conduct comprehensive health and psychosocial evaluations
• Monitor resident conditions and report any changes or concerns
• Coordinate services to promote resident health and well-being
• Collaborate with interdisciplinary teams to ensure seamless care delivery
• Ensure community compliance with federal, state, and local laws and regulations

Requirements:

• Two years of LPN experience required
• Two years of experience in assisted living or nursing home settings
• Licensed Practical Nurse license in good standing
• Ability to work nights and weekends
• Strong organizational, time management, and communication skills
• Ability to handle multiple priorities and delegate assignments
