Job Summary

We are seeking a highly skilled and compassionate Medication Aide to join our team at West Side Federation for Senior and Supportive Housing, Inc. As a Medication Aide, you will play a critical role in ensuring the health and well-being of our residents by providing medication administration and support services.

Key Responsibilities
  • Medication Administration: Ensure that all medications are administered according to medication orders and in compliance with all applicable regulations.
  • Medication Storage: Ensure that all medication is stored appropriately in original labeled containers, in accordance with safety requirements.
  • Medication Orders: Ensure that no medication is dispensed without written orders from the prescribing practitioner.
  • Documentation of Medication Orders: Ensure that the Medication Administration Register (MAR) accurately reflects the written orders of the resident's prescribing practitioner(s).
  • Documentation of Medication Administration: Ensure that all medication administration is documented by the individual assisting with administration and follows all applicable regulations.
  • Medication Supply: Ensure that there is an adequate supply of medications on hand for each resident.
  • Medication Changes: Ensure that changes are made only when there is a prescribing practitioner's written order, or when there is a Confirmation of Telephone Order in place.
  • Incontinence Supplies: Assist residents with monitoring and management of supplies used to assist with incontinence issues.
  • Medication Updates: Create/collect/maintain all Medication Update forms related to resident appointments with outside providers.
Additional Responsibilities
  • Supervise Personal Hygiene: Supervise the personal hygiene and physical well-being of the residents.
  • Assist with Activities of Daily Living: Assist residents as needed in activities of daily living, including bathing, dressing, grooming, toileting, walking, and other personal care needs.
  • Monitor Resident Meals: Monitor to ensure that residents are eating meals regularly each day.
  • Escort Residents: Escort residents to medical and other appointments as directed.
  • Assist with Meal Preparation: Assist kitchen staff as needed in meal preparation, serving, and clean up.
  • Participate in Team Efforts: Work as a member of the team to establish and maintain a high level of care and respect for the West 108th community.
Qualifications
  • Experience: Minimum of 2 years' experience working with dependent persons.
  • Age: At least 21 years of age.
  • Cultural Competence: Able to work in a multi-cultural environment, with staff and residents.
  • Physical and Emotional Ability: Emotionally, mentally, and physically able to perform job responsibilities.
  • Language Skills: Able to speak, read, and write English (as required by DSS regulation).
