Overview
As a Care Coordinator at Covenant Health, you will play a pivotal role in supporting women with high-risk pregnancies. Our mission is to guide patients through their healthcare journey, ensuring they receive the best possible care from conception to delivery.

With over 25 years of experience, we specialize in high-risk pregnancy services, having successfully assisted thousands of families. Our facilities are equipped to handle a variety of complex pregnancy situations, including advanced maternal age, multiple births, and medical complications.

Position Summary:
In this role, you will be responsible for managing patient care across various stages, focusing on quality outcomes and resource coordination. Your efforts will involve:

  • Conducting thorough assessments of patients' physical and emotional needs in collaboration with healthcare professionals.
  • Evaluating care placement options based on clinical conditions and available resources.
  • Identifying opportunities for improvement in service utilization and patient education.
  • Facilitating appointments and ensuring seamless communication among all healthcare providers involved in patient care.
  • Connecting patients with community resources to enhance their health and satisfaction.
  • Participating in interdisciplinary team meetings to strategize patient care and discharge planning.
  • Monitoring patient progress and adjusting care plans as necessary to achieve optimal outcomes.

Qualifications:
Minimum Education: No specific educational requirements; however, candidates must meet the licensing standards for the role.
Minimum Experience: A minimum of three years of clinical experience in a women's health setting is required.
Licensure Requirement: Must hold a current and valid license in good standing in the State of Tennessee.
