Job Summary

We are seeking a highly skilled Medical Navigator to join our team at MedNorth Health Center. This is an exciting opportunity for a dedicated and compassionate individual to play a vital role in providing exceptional patient care.

About the Role

The Medical Navigator will serve as a liaison between patients, care teams, and the Patient Access Team. This critical position will help bridge conversations with patients, remove barriers to accessing same-day or immediate care, and conduct telephonic and/or face-to-face triage to determine patient needs.

In addition to these responsibilities, the Medical Navigator will assess, plan, implement, coordinate, monitor, and evaluate healthcare options available to patients based on their triaged needs. The goal of this position is to eliminate barriers to access and improve health outcomes for identified populations across the practice.

Key Responsibilities
  • Perform patient assessments to determine emergency treatment, appointment scheduling, and other essential services.
  • Initiate emergency treatment when necessary and provide timely interventions using proper nursing techniques, procedures, and safety precautions.
  • Communicate effectively with patients, nurses, physicians, clinical associates, non-clinical staff, supervisors, and department heads regarding patient information and care plans.
  • Document assessment data and make triage decisions, which are communicated to other patient care providers.
  • Ensure that physician orders are transcribed and implemented in a timely manner, adhering to policies and procedures.
  • Administer medications according to proper nursing techniques and procedures, utilizing pharmacology knowledge and available resources.
  • Maintain accurate records of medication administration on the Electronic Medical Record (EMR) in accordance with MedNorth Health Center policies and procedures.
  • Participate in quality improvement programs, monitoring project outcomes, and representing the organization as a liaison to state-related healthcare organizations.
  • Implement and document state/government funding programs, including Title X and Women's Cancer Screening initiatives.
  • Collaborate with the Director of Nursing to support training and development of Medical Assistants (MAs) and Registered Nurses (RNs).
  • Represent the organization with a positive, professional attitude when communicating with patients and visitors.
Requirements
  • Licensed as an RN in the State of North Carolina, functioning within the NC Board of Nursing Nurse Practice Act.
  • Current BCLS certification from the American Heart Association or American Red Cross Healthcare Provider.
  • Three years of experience working in a healthcare facility.
Estimated Salary Range:

$63,000 - $83,000 per year, depending on experience and qualifications.
