POSITION OVERVIEW
The Patient Care Coordinator is responsible for the planning, organization, and management of hospice services, bringing a wealth of nursing experience with a focus on community health education. This role leverages community resources to tailor services that address the unique needs of individuals and families in their own environments. The position reports to the Hospice Administrator.
KEY RESPONSIBILITIES
In addition to the overarching responsibilities outlined in the position overview, the following duties are critical for effective performance in this role:
- Conducts comprehensive initial and ongoing evaluations of patients and families to identify hospice requirements, including thorough physical assessments and medical histories.
- Delivers professional nursing care by applying all aspects of the nursing process.
- Assesses and monitors patient conditions by developing and implementing care plans, regularly reassessing needs, and adjusting plans as necessary.
- Utilizes health assessment data to establish nursing diagnoses.
- Creates care plans that set achievable goals based on nursing diagnoses, incorporating palliative care strategies and involving patients and families in the planning process.
- Initiates preventive and rehabilitative nursing actions, administering medications and treatments as prescribed by the physician.
- Provides guidance to patients and families in addressing nursing and related needs.
- Offers health education to patients as appropriate based on assessments and care plans.
- Assists patients with daily living activities, promoting self-sufficiency and optimal comfort care.
- Acts as the Case Manager when designated by the Clinical Supervisor/Nursing Supervisor, coordinating care for an assigned patient caseload.
- Maintains accurate and relevant clinical documentation regarding patient conditions and care provided, including pain and symptom management updates.
- Communicates with physicians regarding patient needs and condition changes, obtaining necessary orders as required.
- Collaborates with community health professionals to ensure cohesive care planning.
- Educates patients and families on self-care techniques, providing instructions on medications, diet, and other physician-ordered guidelines while recognizing opportunities for health counseling.
- Ensures a safe environment for patients.
- Supports continuity of care by assisting patients, families, and team members.
- Works collaboratively with families and the hospice Interdisciplinary Group to address the emotional needs of patients and families.
- Participates in interdisciplinary meetings.
- Engages in on-call responsibilities as outlined in the on-call policy.
- Ensures availability of necessary equipment and services.
- Supervises support staff and delegates tasks as needed.
- Takes responsibility for personal professional development, maintaining and enhancing nursing knowledge and skills through ongoing education and training.
- Completes assigned case responsibilities as requested.
- Actively participates in quality improvement initiatives and activities.
The above responsibilities represent a summary of the primary duties performed by individuals in this role. Additional job-related tasks may be assigned as necessary.
SUPERVISORY ROLE
- This position does not have direct supervisory responsibilities.
EDUCATION AND EXPERIENCE REQUIREMENTS
- Must be at least 18 years of age.
- Required health screenings, TB clearance, and criminal background checks.
- Must hold a current Registered Nurse license to practice in the state.
- Graduate of a nursing program accredited by the National League for Nursing.
- Current CPR certification is mandatory.
- Must possess a valid driver's license and have access to a reliable, insured vehicle.
- Registered Nurses should have a minimum of one year of professional nursing experience within the last three years, or a baccalaureate degree in nursing from an accredited program and a current RN license. A minimum of two years' experience, with at least one year in public health, home care, or hospice nursing, is preferred.
KNOWLEDGE, SKILLS, AND ABILITIES
To excel in this position, candidates must demonstrate the following:
- Ability to work independently with minimal supervision.
- Management experience is not required, but the role includes supervising hospice aides.
- Strong observational, problem-solving, verbal, and written communication skills; nursing competencies as per checklist.
- Capacity to organize and prioritize workload effectively.
- Physical ability to perform tasks such as prolonged walking or standing, lifting, positioning, and transferring patients, as well as handling supplies and equipment.
- Visual and auditory acuity to perform necessary nursing skills.
