Job Summary:
Covenant Health (Tennessee) is seeking a skilled Registered Nurse to join our Home Health team. As a Home Health Nurse, you will provide professional nursing care to patients in their homes, coordinating care planning with other disciplines and promoting patient independence.
Responsibilities:
- Reviews patients' records, determines home care needs, and develops care and discharge plans.
- Performs and records cardiovascular, pulmonary, neurological, GI, skin, intake and output, and pain assessments.
- Administers medications, provides teaching and training for patients and caregivers, and performs treatment procedures.
- Coordinates and refers other services, participates in case conferences, and acts as liaison between MD and patient/family.
- Completes daily documentation, prepares and submits reports, and follows admission, discharge, on-call, and death procedures.
Requirements:
- Graduation from an accredited school of nursing.
- One year of professional experience in hospital or prior home health experience required.
- Must have and maintain State of Tennessee RN license, CPR, and valid Tennessee driver's license Class D.
