Job Overview:
The Patient Care Coordinator (RN) plays a crucial role in overseeing and managing the daily operations of the Care Management programs at Health Alliance. This position is essential in ensuring the effective utilization of hospital resources while upholding the highest standards of patient care.
Key Responsibilities:
- Supervise the patient evaluation process within the Care Management department to identify relevant patient issues.
- Engage with patients through direct communication with healthcare staff, physicians, and families, including reviewing patient medical histories.
- Assess the suitability of admissions and the severity of illnesses based on established criteria used by various third-party payers.
- Facilitate collaboration with a multidisciplinary care management team to tailor patient care plans, promote efficient resource use, and ensure timely patient discharges.
- Oversee the collection of clinical data based on predetermined quality and outcome indicators set by the medical staff and administration.
- Provide orientation and mentorship to staff, manage scheduling, and conduct evaluations within the Care Management department.
- Perform additional related duties as required within the department.
Qualifications:
Minimum of three years of experience in hospital case management. Current licensure as a Registered Nurse in the State of New York. Familiarity with Medicare/Medicaid criteria and insurance regulations is essential.
Education:
A Bachelor's degree in Nursing is preferred.
Additional Information:
Individuals in this role may encounter human blood and other potentially infectious materials. Adherence to universal precautions and the use of personal protective equipment is required. Understanding infection control policies is essential.
