Responsibilities- Provide Direct Patient Care: Implement and monitor patient care plans, monitor and record patient condition, and communicate with patients and their families as needed.
- Coordinate Patient Care: Serve as a primary coordinator of all disciplines for well-coordinated patient care, including working with physicians, nurses, and other healthcare professionals.
- Assess and Coordinate Discharge Planning: Assess patients' discharge planning needs and coordinate with the healthcare team to ensure a smooth transition to post-acute care.
- Apply Evidence-Based Practice: Apply the existing body of evidence-based practice and scientific knowledge in healthcare to nursing practice, ensuring that nursing care is delivered based on patients' age-specific needs and clinical needs.
- Work in a Constant State of Alertness: Work in a constant state of alertness and safe manner, using your skills and experience to provide high-quality patient care.
- Education: Graduate of an accredited school of nursing or education equivalency for licensing.
- Experience: No experience required.
- License and Certifications: Basic Life Support HealthCare Provider (BLS HCP) - American Heart Association (AHA) and Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR) or equivalent.
