Job Summary

We are seeking a highly skilled and compassionate Residential Case Manager to lead our Level 5 Foster Home. The successful candidate will provide case management, program management, staff supervision, and direct care support to individuals with differing abilities.

Key Responsibilities
  • Provide inter-intra agency case management and develop, update, and implement Individual Support Plans (ISPs) to ensure individuals receive authorized care and services.
  • Maximize social functioning by promoting community integration and providing support and advocacy to safeguard human and civil rights.
  • Provide guidance to direct care staff and support to individuals during challenging behaviors, as well as crisis response as needed.
  • Lead and guide direct care staff through training, coaching, mentoring, and performance management.
  • Provide direct care support to individuals based on program needs and staffing levels.
Qualifications
  • A Bachelor's degree in a human services related discipline, such as social work, sociology, special education, or psychology, and three to five years of experience in case management, program management, staff supervision, and caregiving.
  • Must be at least 21 years of age and maintain a valid driver's license with an excellent driving record.
  • Pass a thorough background check and be mentally and physically capable of assisting supported individuals based on their needs.
  • Be flexible and adaptable in hours of work, environments, and situations.

  • $21.00 to $25.00/hour, plus overtime for hours worked in excess of 40 per week.
