Job Title: Licensed Practical Nurse - Skilled Care Provider

Job Summary:

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Care and Help Home Care, LLC. As an LPN, you will be responsible for providing high-quality patient care in the comfort of their own homes. Your primary focus will be on administering care plans, ensuring compliance with nursing standards, and providing exceptional patient care.

Key Responsibilities:

• Administer care plans in collaboration with the Registry Nurse and Director of Nursing
• Perform duties according to professional standards as delegated by a Registered Nurse supervisor
• Delegate nursing duties to caregivers to safely meet patient needs
• Provide ongoing instructions to home care aides during field visits
• Develop and review personal care plans from all contract/payor sources
• Document services rendered and changes in client conditions

Requirements:

• Current and unencumbered license to practice as a practical/vocational nurse
• Current CPR certification
• Infectious disease clearance for direct patient care
• Reliable transportation and a valid driver's license

Work Environment:

• Frequent degree of travel required
• Ability to freely bend, kneel, sit, and stand for potentially extended periods

About Care and Help Home Care, LLC:

We are a non-skilled home care agency providing services to individuals in their homes or other independent living environments. Our services include assistance with self-administration of medications, personal care, homemaking, transportation, companionship, and respite care. We conduct criminal background checks and physicals for all direct care workers and office staff with direct consumer contact.
