Job Description

This Mental Health Advocate position plays a critical role in our Transitional Living Apartment Treatment Program, providing essential support services to individuals with severe mental illnesses.

You will work closely with program participants, their families, and other stakeholders to develop and implement individualized plans that promote recovery, independence, and community integration.

Responsibilities include:

  • Providing casework and Medicaid billing services for program participants.
  • Conducting regular visits to program participants in their apartments and common areas.
  • Developing and implementing individualized plans to promote recovery and independence.
  • Collaborating with other agencies and organizations to ensure seamless service delivery.
Requirements
  • Bachelor's degree in Human Services or related field; or Associate's degree with 3 years of experience in mental health settings.
  • Valid New York State Driver's License with good driving record and access to reliable vehicle for travel and client transportation.
