SUMMARY:
The Care Coordinator plays a crucial role in advancing the mission and vision of Go Kids Inc. This position is responsible for overseeing essential activities related to integration and navigation, which includes outreach, engagement, screening, care coordination, and participation in multi-disciplinary review teams.
The Care Coordinator collaborates closely with families, county agencies, and community partners to create integrated and coordinated family success plans.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Establish connections with community-wide, multi-disciplinary service providers, including educators, primary care physicians, and therapists.
- Coordinate with resource and referral entities to ensure current information on available resources, programs, and services for young children and their families.
- Collaborate with internal staff and community partners to enhance referral processes to and from Go Kids services.
- Participate in monthly meetings with Family Services Teams to improve referrals and care coordination.
- Provide information about community supports and services available to children and families.
- Utilize relationship-based strategies that respect family language, culture, and context to understand their strengths and goals.
- Conduct appropriate child and family screenings and manage the informed consent process.
- Facilitate weekly triage meetings with internal and external partners.
- Review recommendations from internal staff and partner agencies with families to offer comprehensive referrals.
- Develop a family-driven Family Success Plan that includes resources, referrals, timelines, and goals.
- Ensure follow-through on referrals to assist families in accessing recommended services.
- Provide case management for families with diverse or complex needs.
- Foster collaboration and communication among service providers through case review meetings.
General Responsibilities:
- Maintain office hours as per the monthly calendar.
- Attend required trainings, meetings, and community events.
- Engage in evaluation and continuous quality improvement activities.
- Participate in professional development and community collaboration.
- Maintain client files in compliance with HIPAA regulations.
QUALIFICATIONS:

EDUCATION and EXPERIENCE:
A Bachelor's degree in Social Work, Psychology, Sociology, Child Development, or a related field is preferred. Candidates should have 2-3 years of experience working with children and families or an equivalent combination of education and experience.
Endorsement in Infant Family Early Childhood Mental Health or relevant coursework is a plus. Candidates must pass a fingerprint and criminal clearance and possess a valid driver's license.
LANGUAGE SKILLS:
Bilingual/Bicultural in Vietnamese/English or Spanish/English is preferred. Candidates should be able to read, analyze, and interpret business documents and regulations.
MATHEMATICAL SKILLS:
Ability to apply mathematical concepts to practical situations.
REASONING ABILITY:
Ability to define and solve practical problems and interpret various instructions.
PHYSICAL DEMANDS and WORK ENVIRONMENT:
Physical demands include talking, hearing, and using hands to handle objects. The employee may occasionally lift up to 25 pounds.
The work environment is usually moderate in noise level.
