Hospice Case Manager Opportunity in Gwinnett County

We are partnering with a reputable hospice organization in Gwinnett County, Georgia, to find an experienced Registered Nurse (RN) to fill a Hospice Case Manager position. This role is ideal for a compassionate and skilled RN who is dedicated to providing high-quality patient care and support to patients and their families.

Responsibilities:
  • Maintain a caseload of approximately 12-14 patients
  • Provide in-home patient care and support
  • Collaborate with the healthcare team to develop and implement individualized care plans
  • Ensure timely and accurate documentation of patient information
Requirements:
  • Registered Nurse (RN) licensure in the State of Georgia
  • 2-3 years of experience in hospice or palliative care
  • 2 years of experience in in-home patient care
  • Familiarity with EMR systems, preferably MatrixCare
Benefits:
  • Annual salary range: $80,000-$90,000+
  • Health insurance with employer-paid COBRA
  • Dental and vision insurance with employer-paid COBRA
  • Mileage reimbursement: $0.48 per mile
  • PTO: 14 days
  • Paid holidays: 7 federal holidays
  • 401(k) with 3% match
