Position Overview

As a Licensed Practical Nurse (LPN), you will provide essential care to residents in a facility dedicated to long-term care, memory support, short-term rehabilitation, and independent living for seniors.

Role Responsibilities

We are seeking a dedicated LPN to join our team in a facility that focuses on long-term care, memory care, short-term rehabilitation, and independent living for seniors. This role is full-time and onsite, with both day and night shifts available.

Key Duties Include:

  • Administering medications and treatments according to established standards.
  • Delivering safe and efficient personal care to residents.
  • Utilizing recommended safety devices and adhering to infection control protocols.
  • Responding promptly to call lights and residents' requests for assistance.
  • Upholding residents' rights at all times.
  • Documenting medications, treatments administered, and care provided.
  • Assisting in the orientation of new staff members.
  • Participating in resident care planning and updating care plans.
  • Assessing residents, documenting findings, and communicating with physicians.
  • Reporting changes in residents' conditions to appropriate parties.
  • Managing the destruction and documentation of discontinued medications.
  • Maintaining accurate records of controlled substances during shifts.
  • Completing necessary documentation including nursing notes and transfer forms.
  • Assisting with unit operations, including monthly record changes.
  • Assigning tasks to nurse aides and providing shift reports.
  • Scheduling and assisting with medical appointments for residents.
  • Administering and recording employee health testing results.

Qualifications Required:

  • Valid Indiana Professional License or a license from an NLC participating state.
  • Proficiency in computer skills and the ability to learn electronic medical record systems.
  • Current American Heart Association CPR and BLS Certification.
  • Completion of a 6-hour Dementia certification prior to starting.
  • Previous experience in long-term care settings.

Preferred Qualifications:

  • Experience with electronic medical records, particularly Matrix Care.
  • Familiarity with memory care environments.
  • Background in short-term rehabilitation.

Physical Requirements:

  • Ability to perform essential job functions in accordance with ADA and other standards.
  • Sedentary work that may involve sitting for extended periods with occasional movement.
  • Capability to perform repetitive tasks on a computer, using a mouse, keyboard, and monitor.
