Position Overview
As a Licensed Practical Nurse (LPN), you will provide essential care to residents in a facility focused on long-term care, memory support, rehabilitation, and independent living for seniors.
Key Responsibilities
- Administer medications and treatments according to established protocols and standards.
- Deliver safe and efficient personal care to residents.
- Implement recommended safety measures and adhere to infection control practices.
- Respond to call lights and assist residents with their needs.
- Uphold residents' rights consistently.
- Document medications administered, treatments provided, and care rendered.
- Assist in orienting new staff members on the unit.
- Engage in resident care planning, updating care plans, and assigning tasks to aides.
- Assess residents, document observations, and communicate findings to physicians; transcribe medical orders.
- Notify resident sponsors of any changes in condition.
- Properly dispose of and document discontinued medications.
- Maintain accurate records of controlled substances during shifts.
- Complete necessary documentation including nursing notes and transfer forms.
- Support the operation of the unit, including monthly record updates.
- Assign and distribute tasks to nurse aides.
- Provide reports to the incoming shift.
- Schedule and assist with medical appointments for residents.
- Administer and document results of employee health tests.
Qualifications
Required:
- Valid Indiana Professional License or a license from a participating NLC state.
- Proficiency in computer skills and the ability to learn electronic medical record systems.
- Current CPR and BLS certification from the American Heart Association.
- Completion of a 6-hour dementia certification prior to employment.
- Experience in long-term care settings.
Preferred:
- Familiarity with EMR systems, particularly Matrix Care.
- Experience in memory care environments.
- Background in short-term rehabilitation.
Physical Requirements
- Ability to perform essential job functions in accordance with ADA and other applicable standards.
- Sedentary work involving prolonged periods of sitting with occasional movement for meetings.
- Capability to perform repetitive tasks on a computer using a mouse, keyboard, and monitor.
