Job Summary

We are seeking a highly skilled and compassionate Registered Nurse to join our team at the San Antonio State Hospital. As a Direct Care RN, you will play a vital role in providing high-quality patient care and contributing to the overall well-being of our residents.

Key Responsibilities
  • Provide routine level nursing care to residents, including assessment, treatment, and documentation of healthcare needs and issues.
  • Evaluate and treat healthcare needs and issues, and report to the appropriate person(s) as needed.
  • Communicate effectively with direct care staff, physicians, administration, and other nursing staff to ensure optimal care as part of the interdisciplinary team.
  • Notify the supervisor and PCP of trends and concerns to ensure consistent, high-quality care for all residents.
  • Complete data entry, required documentation, and reports, and maintain accurate records.
  • Assist with services provided by LVNs and Certified Medication Aides (CMA), as needed, and make recommendations for improvements.
  • Participate in facility quality assurance/quality improvement (QA/QI) initiatives, as required.
  • Respond to emergency situations to ensure safety and care of residents and staff.
Requirements
  • Licensed to practice as a registered nurse in the State of Texas (or a state that recognizes reciprocity through the Nurse Licensure Compact).
  • At least six (6) months of experience as a practicing registered nurse in a health agency, hospital, or healthcare facility is preferred.
  • Bachelor's Degree is preferred.
