Job Title: Transitions of Care Clinical Advocate RN

Job Description

In this critical role, you will be responsible for coordinating patient discharges, providing education on self-care and medication management, and facilitating communication between healthcare providers and patients.

Key Responsibilities
  • Work closely with patients and their families to develop personalized care plans
  • Coordinate with case management teams to prevent readmissions and ensure seamless transitions of care
  • Develop and implement strategies to improve patient outcomes and satisfaction
  • Maintain accurate and up-to-date records and documentation
  • Communicate effectively with multidisciplinary teams, including physicians, nurses, and social workers
Qualifications

To be successful in this role, you will need:

  • A minimum of 2 years' experience in primary care or ambulatory settings with a focus on patient navigation and transitions of care
  • An unrestricted RN license in the State of Texas and current Basic Life Support certification
  • Experience with Epic electronic health record system and training in Epic end-user programs preferred
Estimated Salary Range

$80,000 - $110,000 per year, depending on experience and qualifications.
