Job Summary:

  • Evaluate post-discharge member needs through assessments and identify opportunities for quality and cost-effective care.">
  • Conduct medication reviews for pre-admission and post-discharge reconciliation to ensure seamless transitions.">
  • Collaborate with care management teams to provide transition support services and develop personalized care plans.">
  • Assess member health status, resource needs, and treatment plans to deliver targeted interventions.">
  • Facilitate active care management based on member needs and provide education and resources to promote wellness activities.">
  • Maintain accurate records of member information and care management activities to ensure compliance with regulatory requirements.">
  • Provide feedback to leadership on opportunities to enhance care delivery and quality outcomes.">

Requirements:

  • ">
  • Masters degree in Behavioral Health or Social Work or a Degree from an Accredited School of Nursing required.">
  • LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN license required.">
  • 2-4 years of related experience preferred.">

Compensation Package:

">

We offer a competitive salary range of $54,000.00 - $97,100.00 per year, along with a comprehensive benefits package that includes competitive pay, health insurance, 401K and stock purchase plans, tuition reimbursement, paid time off plus holidays, and a flexible approach to work with remote, hybrid, field or office work schedules. Total compensation may also include additional forms of incentives.
