Nurse Practitioner or Physician Assistant

INTEGRITUS HEALTHCARE MGT SERVICES, INC is seeking a skilled and dedicated Nurse Practitioner or Physician Assistant to join our team at our Skilled Nursing Facility. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and collaborating with our interdisciplinary team to ensure the best possible outcomes for our patients.
Key Responsibilities- Participate in the medical evaluation of patients admitted to our Skilled Nursing Facility, including evaluating and documenting medical necessity for specific levels of care.
- Offer advice and information to staff and referring physicians on medical interventions consistent with the medical plan of care.
- Consult with attending physicians and referral sources as requested to ensure comprehensive medical care for patients is effectively and expeditiously accomplished.
- Provide information to patients and families regarding their medical condition and plan of care.
- Participate in the review and development of practice protocols in the facility.
- Responsible for informing primary physicians and referral sources of the outcome of Nurse Practitioner consultations and evaluations.
- Responsible for documenting interactions between nursing staff, referral sources, patients, and families.
- Maintain regular contact with referral sources as assigned.
- Complete, maintain, and submit accurate and relevant clinical notes regarding patient condition and care.
- Minimum of two (2) years of professional experience in healthcare.
- Experience in a skilled nursing facility preferred.
- Experience in acute care in an institutional setting preferred.
- Nurse Practitioner or Physician Assistant licensure required.
- Possesses and maintains current CPR Certification.
- Current Massachusetts licensure: Nurse Practitioner or Physician Assistant.
- Valid driver's license.
- May be exposed to infections and contagious diseases.
- Contact with patients under a wide variety of circumstances.
- May be exposed to/occasionally exposed to patient elements.
- Subject to varying and unpredictable situations.
- Handles emergency or crisis situations.
- Travel required.
