Job Summary

The Detroit Wayne Integrated Health Network is seeking a highly skilled Clinical Care Coordinator to join our team. As a Clinical Care Coordinator, you will play a critical role in ensuring that consumers receive the appropriate level of care by completing telephonic clinical screenings and assessments.

Key Responsibilities
  • Telephonic Clinical Screenings: Conduct thorough clinical screenings and assessments to determine eligibility for public mental health services.
  • Referral and Authorization: Determine appropriate levels of care for referral, assist clients in selecting service providers, and initiate referrals to selected providers.
  • Re-authorization and Monitoring: Re-authorize SUD/Mental Health/co-occurring services, monitor client compliance with services, and assist with aftercare/recovery plan services.
  • Client Communication: Communicate with clients, providers, and community agencies to ensure seamless care coordination.
  • Data Management: Enter data and reports into electronic databases, track and monitor cost factors, and identify trends at the provider and network level.
Requirements
  • Education: Master's Degree in Social Work, Psychology, Counseling, Nursing, or a related field.
  • Experience: Three years of professional clinical experience in behavioral healthcare or a community mental health setting.
  • Licensure: Valid State of Michigan clinical licensure (RN, LMSW, LMHC, LPC, LLP, or PhD).
Working Conditions

Part-time staff are allowed to work remotely with management approval.
