Kent Hospital is seeking a highly skilled Ambulatory Staff Pharmacist Per-Diem to join our team. As a key member of our healthcare team, you will provide integrated support to patients and physicians by delivering a full continuum of medication management services.
Key Responsibilities- Prepare medications and provide pharmacological information to patients and healthcare professionals.
- Monitor patient drug therapies and provide adherence support to ensure optimal health outcomes.
- Collaborate with healthcare teams to develop and implement medication management plans.
- Provide education and training to patients and healthcare professionals on medication use and safety.
- Participate in quality improvement initiatives to enhance patient care and safety.
- Doctor of Pharmacy (PharmD) or BS in Pharmacy degree.
- Current licensure as a Pharmacist in the State of Rhode Island.
- At least 5 years of experience in a specialty or retail pharmacy setting, with a preference for 8+ years.
- Advanced certification in areas such as immunization, medication therapy management, diabetic education, or cardiovascular education is preferred.
