Ambulatory Clinical Pharmacist Job Description

At Kent Hospital, we are seeking a highly skilled Ambulatory Clinical Pharmacist to join our team. As a key member of our interdisciplinary care team, you will provide evidence-based, patient-centered medication recommendations to optimize medication regimens and promote high-quality, cost-effective pharmaceutical care.

Key Responsibilities:
  • Reconcile and assess medication regimens for adherence and optimal therapy to achieve desired outcomes
  • Provide evidence-based, patient-centered medication therapy recommendations as part of the interdisciplinary care team
  • Provide medication and disease state management education to patients
  • Collaborate with providers and patients to ensure high-quality, cost-effective care
  • Document all encounters within the electronic health record(s) as well as other platforms as necessary
Requirements:
  • Doctor of Pharmacy (PharmD) degree with 2 years of professional experience or Bachelors of Science in Pharmacy with 3 years experience
  • Current RI Pharmacist license is required
  • PGY-1 Residency or Board of Pharmaceutical Specialties certification preferred
  • Medication Therapy Management certificate required within 6 months of hire
