Competitive Compensation Package
The estimated annual salary for this position is $75,000 - $95,000, depending on experience and qualifications. Job Responsibilities
The successful candidate will be responsible for administering skilled nursing care to patients in their place of residence, under the supervision of a registered nurse. This role requires strong clinical documentation skills, good communication skills, and the ability to work independently as part of a interdisciplinary team. Key responsibilities include:
  • Assisting in identifying patient physical, psychosocial, and environmental needs
  • Participating in planning and implementing care in conjunction with the RN, in accordance with the POC
  • Providing care utilizing infection control measures that protect both staff and patient (OSHA)
  • Recording treatments administered in medical record
  • Recording clinical documentation according to accepted regulatory, corporate and professional guidelines

Qualifications
To be considered for this position, candidates must have a valid LVN license, good assessment skills, and strong clinical documentation skills. They must also be able to read, write, and speak English fluently. Reliable transportation and a valid driver's license are also required.
