We are seeking a skilled Registered Nurse to provide high-quality healthcare services to patients in their homes.

Responsibilities
  • Assess and evaluate patient needs/problems, identifying mutually agreed-upon goals for care plans with patients.
  • Report patient status, updates, and discharge planning process to clinical leadership, attending physician, and hospice physician.
  • Attend staff meetings and educational in-services as needed.
  • Participate in Quality Assessment and Performance Improvement (QAPI) program through submission of data collection.
Requirements

To be successful in this role, you will need:

  • A current unencumbered registered nurse license in the state of practice or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC).
  • The ability to work in a field setting and demonstrate sound nursing judgment.
  • Experience with Microsoft Office.
Benefits

We offer a competitive salary range of $65,000 - $85,000 per year, depending on experience, qualifications, and location. Additional benefits include:
