Position Overview:
We are seeking a dedicated Licensed Practical Nurse (LPN) with experience in a medical office setting. The ideal candidate will possess the ability to build respectful relationships with clients, aiding them in developing skills and confidence while fulfilling service objectives.
Key Responsibilities:
- Support the Medical Services Coordinator in managing medical services, scheduling, and documenting physician and psychiatrist appointments.
- Oversee clinic operations for residents in compliance with state regulations.
- Provide nursing care to residents as directed by the Medical Services Coordinator and in accordance with the policies of the Youth Opportunity Center.
- Ensure that health records are kept confidential and comply with regulatory standards, including initial and periodic health assessments, dental care, psychotropic medication management, immunizations, and vision examinations.
- Document all observations and incidents, including any health-related issues or potential abuse.
- Facilitate follow-up actions by program staff regarding medical concerns, medication orders, care plans, and procedural developments.
- Collaborate with the contracted pharmacy to ensure timely and accurate medication orders, including assisting with prior authorizations and reviewing generic medication information for billing purposes.
- Adhere to medication policies to guarantee safe and efficient practices in obtaining, preparing, administering, documenting, and disposing of medications, in line with Youth Opportunity Center policies.
- Maintain clinic hours and participate in an on-call rotation to address the medical needs of residents.
- Provide guidance to program staff regarding necessary actions, which may include referrals for off-site medical care.
- Maintain a comprehensive database of all medical records on campus and ensure accurate input of required information into the client software system.
- Conduct initial medical assessments within one business day and ensure physical examinations are completed within five business days for all residents.
- Assist in the Medical Services Training for all direct care staff as directed by the Medical Services Coordinator.
